Anesthesia Plenary: Anesthesia for Longer Office-Based Cases: Pearls and Pitfalls

OMSs perform office-based surgical and restorative cases that may take several hours. Patients who are older or have additional health concerns require more careful selection to ensure they can withstand several hours of office-based anesthesia and/or immediate prosthesis conversion. This session will examine pearls and pitfalls in the selection of such patients, modification of anesthetic techniques and recommendations to prevent complications such as urinary retention and deep vein thrombosis. Attendees also will learn from mistakes made from closed claims summaries of such complications.

Learning Objectives

At the conclusion of this program, participants should be able to:

  1. Determine selection criteria for longer office-based anesthetics.
  2. Analyze various techniques for management of longer office-based anesthetics.
  3. Explain the complications that can occur during longer anesthesia cases in the office setting.
  4. Evaluate the medico-legal implications of performing longer procedures under anesthesia in the office.
  5. Discuss if engaging a second provider is indicated in this setting.

Pursuing her passion for surgery, she completed an extensive and rigorous residency program in Oral and Maxillofacial Surgery at Mount Sinai Hospital in New York in 2018, later obtaining her board certification in 2020. Currently, Dr. Patel practices in her own establishment at Harmony Oral and Facial Surgery in Tega Cay, SC. She, also, holds hospital privileges at Piedmont Medical Center in South Carolina.

Bryant Cornelius, DDS, MBA, MPH

Dr. Cornelius received his DDS degree from Northwestern University, his MBA degree from the University of Utah, and his MPH degree from The University of Pittsburgh. After practicing general dentistry for 15 years, Dr. Cornelius completed a three-year residency in anesthesiology at the University of Pittsburgh. He is a Fellow of the American Dental Society of Anesthesiology and a Diplomat of both the American Dental Board of Anesthesiology and the National Dental Board of Anesthesiology. Dr. Cornelius is the Medical Officer and head anesthesiologist for The Cleveland Children’s Surgery Center and is currently serving as the President-Elect of the American Society of Dentist Anesthesiologists. Dr. Nicholas J. Bournias is a practicing oral and maxillofacial surgeon in the Detroit metropolitan area. After completing his undergraduate education at Oakland University in Michigan, he received his dental degree from the University of Detroit in 1990. Dr. Bournias completed his Oral and Maxillofacial Surgery residency at Henry Ford Hospital in Detroit, Michigan in 1994. In addition to his private practice, he is the Chief of Oral and Maxillofacial Surgery at Oakland University William Beaumont School of Medicine, and a member of the Craniofacial team. Dr. Bournias has been a Diplomate of the American Board of Oral and Maxillofacial Surgery since 1996. He has served as a committee member of the Michigan Dental Association Insurance Financial Group for four years. He has held the past position of State of Michigan Oral and Maxillofacial Surgery Specialty Board Examiner and has also served on the American Board of Oral and Maxillofacial Examination Committee for seven years. He is a past member on the State of Michigan Board of Dentistry. Dr. Bournias is the former Co-Chair of the Cleft Lip and Palate Clinic at the Hurley-Mott Hospital in Flint, Michigan. Working in various capacities with OMSNIC, he has been a member of the OMSNIC Advisory Board, the Underwriting Appeals Committee, the Underwriting Committee and Claims Committee. He has been a Director of the Company since 2016 and of Fortress beginning in 2017. He has also served as a Director of The River Agency since 2017.

Graduating from Columbia’s School of Dental Medicine and completing advanced training in oral surgery at North Shore’s Long Island Jewish Medical Center before earning a medical degree from Stony Brook University, Dr. Neugarten’s
areas of expertise cover an array of advanced treatments from individually customized orthognathic plates to robotically guided implant reconstructions. A distinguished fellow of the American College of Surgeons.
Dr. Neugarten has also served as an educator of residents and students at Cornell-Weill and Long Island Jewish Medical Centers as well as lecturing internationally on the latest modalities in oral and maxillofacial surgery. He additionally established the Zaga Center New York City, a collaborative effort within the international Zaga community of surgeons dedicated to advancing the field of zygomatic reconstruction.
