Avoiding Nerve Injury in Oral and Maxillofacial Surgery

Nerve injuries in oral and maxillofacial surgery are not uncommon. Yet many institutions do not have a trigeminal or facial nerve repair specialist, so prevention of these nerve injuries is of the utmost importance. This session will address the complexity, etiology, epidemiology and psychosocial aspects of injury to the trigeminal or facial nerve in oral and maxillofacial surgery and provide an understanding of preoperative and intraoperative adjuncts and techniques to avoid injury to these nerves. Discussions will expand beyond nerve injury as a result of dentoalveolar surgery and will include other types of surgery including orthognathic, trauma and temporomandibular joint surgery.

Learning Objectives

At the conclusion of this program, participants should be able to:

  1. Explain the complexity, etiology, epidemiology and psychosocial aspects of injury to the most common sensory and motor nerves encountered in oral and maxillofacial surgery.
  2. Determine preoperative and intraoperative adjuncts and techniques to avoid nerve injury.
  3. Describe the clinical effects of injury, diagnosis and histopathologic classifications of trigeminal and facial nerve injury.
  4. Discuss the role of informed consent in patients at risk of trigeminal and facial nerve injury.

Dr. Agarwal is an Assistant Professor at Virginia Commonwealth University in the Department of Oral & Maxillofacial Surgery. He completed his residency program at Parkland Memorial Hospital/UT Southwestern Medical Center in Dallas, Texas. He is actively involved in teaching and training residents and dental students at Virginia Commonwealth University. His clinical and research area of focus includes the prevention and management of nerve injury which he received extensive training in during residency at UT Southwestern Medical Center. Dr. Agarwal hopes to encourage future dental students to pursue careers in OMS and to continue to train the future of our specialty.

Dr. Renapurkar is an Associate Professor and Director of the residency program in Department of Oral and Maxillofacial Surgery (OMFS) at Virginia Commonwealth University. He received Doctor in Dental Medicine (D.M.D) degree at Boston University. Prior to this, he completed a Bachelor of Dental Surgery degree at Government Dental College and Hospital, Hyderabad, India. He then completed a residency program in Oral and Maxillofacial Surgery at University of Minnesota followed by a fellowship in Pediatric Oral and Maxillofacial Surgery at Massachusetts General Hospital prior to joining VCU as a full-time faculty. 

Dr. Renapurkar has published several peer reviewed articles, book chapters, as well as lectured at local study clubs, state and national professional meetings. He is a Diplomate of the American Board of Oral and Maxillofacial Surgery, a Fellow of the American Association of Oral and Maxillofacial Surgeons, American College of Surgeons and American Society of Temporomandibular Joint Surgeons. He has served as the past president of the Virginia Society of Oral and Maxillofacial Surgeons, member of AAOMS Committee on Research Planning and Technology Assessment and examiner in the American Board of Oral and Maxillofacial Surgery. He is involved with undergraduate and graduate OMFS education, and has been an active speaker at various local, regional, national events.
