Obstructive sleep apnea (OSA) is associated with several medical comorbidities. Continuous positive airway pressure (CPAP) is an effective treatment that can alleviate airway obstruction. Yet over 50 percent of patients refuse to wear the mask or use it consistently. The hypoglossal nerve stimulator was introduced recently as a promising therapy for OSA. This course will outline the multidisciplinary team approach to managing OSA, the placement and activation of the hypoglossal nerve stimulator. Discussions will include the indications, contraindications and complications associated with the device implantation from surgical and sleep medicine perspectives. In addition, the session will look at the journey for patients with hypoglossal nerve stimulators from diagnosis to activation and the subsequent device adjustment, if indicated.
Learning Objectives
At the conclusion of this program, participants should be able to:
- Evaluate the concept of nerve stimulation in the treatment of OSA.
- Analyze literature to understand the criteria of success for the hypoglossal nerve stimulator.
- Discuss the perioperative workup, indications and contraindications of the hypoglossal nerve stimulator.
- Explain the surgical procedure for inserting the hypoglossal nerve stimulator therapy and how to minimize the complications.
- Define the role of the sleep medicine team in activating the implant, titrating the therapy and managing persistent OSA.

Dr. Marwan is an associate professor of surgery and the division chief at the University of Texas Medical Branch. Dr. Marwan completed his postdoctoral training in oral and maxillofacial surgery at the University of Miami/ Jackson Memorial Hospital. He then completed two years of fellowship training in Head and Neck Oncology and Microvascular Surgery. In 2018, he joined the faculty at the University of Texas Medical Branch at Galveston. Dr. Marwan is double board certified in oral and maxillofacial surgery, oral, head and neck oncology, and microvascular surgery. Dr. Marwan has authored several manuscripts in Head and Neck Oncology, reconstruction, and craniomaxillofacial trauma. In addition, he wrote and edited multiple book chapters on CMF and maxillofacial surgery.

Rizwana Sultana, MD

Dr. Sultana is a Sleep Medicine Specialist, trained in general pediatrics and adult/pediatric sleep medicine. She is an Associate Professor in the Department of Internal Medicine. She completed her Pediatric Residency at Texas Tech University Health Sciences Center in El Paso and her Sleep Medicine Fellowship from Baylor College of Medicine in Houston, TX.
Dr. Sultana has research interests in sleep apnea and obesity in children, insomnia and behavior problems in children, narcolepsy, circadian rhythm problems, and parasomnias.
