Insurance companies use different methods and policies in determining claims processing based on coding guidelines and medical necessity. CMS created the National Correct Coding Initiative (NCCI) to promote national correct coding methodologies and to control improper coding leading to inappropriate payment. Today, multiple parts of the NCCI edits play a role in payment or denial of Medicare claims. In addition, many commercial insurance payers also have implemented these edits in their own reimbursement systems. With an understanding of the edits and how to apply them, denials can be corrected for timely reimbursement, which is critical because insurance carriers allow corrections and reconsiderations for only a specified time after the date of service or original filing date. James C. Melville, DDS, FACS, is an Associate Professor in the Department of Oral and Maxillofacial Surgery at the University of Texas Health Science Center at Houston UTHealth and specializes in Oral, Head & Neck Oncology, and Microvascular Reconstructive Surgery. He graduated from the University of Michigan School of Dentistry in Ann Arbor, Michigan, in 2005 and completed a General Practice Residency at the University of California San Francisco in 2006. He completed a residency in oral and maxillofacial surgery in 2012, followed by fellowships in oral, head, and neck oncology and microvascular reconstructive surgery at the University of Miami/Jackson Memorial Hospital. He was appointed to the UTH OMFS internship director in 2017.
Dr. Melville was awarded the Faculty Educator Development Award (FEDA) in 2019 by the American Association of Oral & Maxillofacial Surgeons (AAOMS). An award given to promising young academic surgeons. He was awarded the AAOMS Advocacy Challenge Coin by the AAOMS Board of Trustees and AAOMS Committee on Government Affairs in 2020. AAOMS designed its own Challenge Coin to recognize oral and maxillofacial surgeons who have made significant advocacy contributions on behalf of the Association. The AAOMS Challenge Coin is awarded to those elite few who transcend the efforts of their colleagues to further the specialty through advocacy. Dr. Melville serves as the Vice-President of the Oral & Maxillofacial Section of the American Association for Dental, Oral, and Craniofacial Research (AADOCR) / International Association of Dental Researchers (IADR) and will be become the President in 2023. He was appointed the Vice President of the AAOMS Clinical Interest Group (CIG) in Neurologic Disorders 2021. He also serves on the International Academy of Oral Oncology (IAOO) as a scientific committee member for its 2022 conference.
Dr. Melville is a diplomate and examiner of the American Board of Oral and Maxillofacial Surgery and a fellow of the American Association of Oral and Maxillofacial Surgeons, the American Academy of Craniomaxillofacial Surgeons, American College of Oral & Maxillofacial Surgeons, and the American College of Surgeons. He has been an invited visiting professor and lecturer at over 15 academic institutions including Case Western University, Columbia University, Harvard/MGH, Emory University, Johns Hopkins University, University of Michigan, UOP/ Stanford University UCLA, USC, and UCLA.
He has written a Springer textbook on maxillofacial reconstruction and contributed to 25 chapters in textbooks concerning oral and maxillofacial surgery, oncology, maxillofacial reconstruction, microvascular reconstruction, and tissue engineering. In addition, he is the author of more than 40 articles in peer-reviewed journals. Ed Braly, DDS, was a member of the University of Oklahoma College of Dentistry’s first graduating class and completed his OMS residency at the University of Oklahoma College of Medicine. He practiced privately in Oklahoma for 35 years before returning to OU College of Dentistry to teach. Dr. Braly has strong interests in facial cosmetic surgery and TMJ surgery. He received Outstanding Faculty Awards in 1998-99, 1999-2000, 2000-01 and 2016-17. Dr. Braly has presented internationally at multiple AAOMS meetings and at the International Federation of Dental Anesthesiology Societies on patient safety and office accreditation.

Richard D’Innocenzo, DMD, MD, is a Clinical Professor and past Director of Predoctoral Oral Surgery Education at Boston University Henry M. Goldman School of Dental Medicine. He is a fellow of AAOMS and a Diplomate of ABOMS. His clinical interests include anesthesia, implantology, simulation training and predoctoral education.
Rachel Uppgaard, DDS, FACS
Rachel Uppgaard, DDS, FACS, is a Clinical Associate Professor in the Division of Oral and Maxillofacial Surgery at the University of Minnesota. Dr. Uppgaard previously worked in private practice and also served as a staff surgeon at the Minneapolis VA Medical Center. She is currently a full-time attending OMS at the University of Minnesota and serves as the Interim School of Dentistry Public Health Liaison. Her interests include TMJ surgery, trauma surgery, orthognathic surgery, pathology, dental implants and severe odontogenic infections. She currently leads research projects on Fanconi anemia, LVADs, odontogenic infections and teaching local anesthesia.

It affords each practitioner their best opportunity to be proactive in risk minimization, by preventing exacerbation of existing disorders and avoidance of new morbidities.
Unexpected “anesthetic challenges” can quickly exhaust patient resilience – the ability to tolerate hypoxia and rate and pressure changes. Patients selected on the basis of reserve – the ability to compensate for increases in functional demand meted by adversity - should be better suited to withstand physiologic insult.
This relevant presentation challenges concepts relating to the accrual of medical history, focused patient assessment, provider ability and efficacy of anesthetic team functioning, which will serve as guides for appropriate selection of anesthetic technique. The format easily affords further discussion on morbidities and mortalities related to office-based anesthesia.
Clinically relevant issues of common medical and anatomic disorders are visited, with emphasis on their impact on choices relating to surgery, anesthesia and location of care. These include concepts of “medical clearance”, as well as evaluation of patients with cardiovascular, pulmonary, psychiatric, endocrine, metabolic, airway and age-related compromises, among others. Suggestions for pre-anesthetic medication management are also provided. Particular emphasis is placed on the airway.

At the conclusion of this program, participants should be able to:
- Discuss clinically relevant issues of common medical problems affecting the major organ systems and explain their impact on pre-anesthetic and peri-anesthetic decision making.
- Identify remediate gaps in knowledge, airway skills and office/staff emergency preparedness to serve as a basis for patient selection and depth of anesthesia limit-setting.

Dr. Robert Bosack was born and raised on the south side of Chicago and maintains his practice in this area. He graduated from the University of Notre Dame in 1974 and received his DDS from Loyola University School of Dentistry, magna cum laude. He received his advanced training at the Cook County Hospital, where he gained vast surgical experience. He has been practicing the full scope of oral and maxillofacial surgery since 1982, with current emphasis on dental implant technology. Dr. Bosack is a diplomate of the American Board of Oral and Maxillofacial Surgery. In addition to his full-time private practice, Dr. Bosack is a Clinical Assistant Professor at the University of Illinois College of Dentistry. He lectures to his peers at local, state and national levels on topics related to surgery, anesthesia and medicine. He is executive director of Dental Anesthesia Online, an online resource for continuing education in dental anesthesia. He is a co-editor of Anesthesia Complications in the Dental Office, published by Wiley Blackwell in 2015.
